Core fields
- Full name, phone and email
- Preferred contact method and time
- Type of matter
- Brief description in the person's words
- Other parties involved
- Any upcoming deadline or court date
Keep it short
Long forms lose people. Ask for what you need to decide on a callback and run conflicts.
Protect confidentiality
Add a notice that submitting the form does not create an attorney-client relationship and that sensitive details should wait. Have counsel approve the wording.
